Before you submit

  • Include the property address and your unit or suite number.
  • Describe what you are seeing, where it is, and when it started.
  • A photo helps. Keep it under 1.2 MB.
  • Choose an urgency that matches the condition, not a guess about response time.

Safety emergencies

If there is a fire, a gas smell, a structural danger, or anyone is hurt, leave the area and call 911. This form is not an emergency line and is not monitored around the clock. For a non-emergency request, you can also call 419-370-0125.
